This advance health care directive is a form of the Power of Attorney which authorizes a health care agent to make decisions regarding the principal's health care, personal care, anatomical gifts, funerals, autopsies, and health related matters. Effective as of July 1, 2000, the California Probate Code requires the use of Statutory Advance Health Care Directive Form. The directive allows you to have legal control over your health care decisions in the event you are unable to decide for yourself.
